ABSTRACT

Objective:To explore the association of bone resorption marker β carboxyterminal peptide of collagen Ⅰ (β-CTX) with hypercalcemia in patients with Graves′ disease (GD).Methods:287 patients with GD who were hospitalized in the endocrinology department of Fuyang People′s Hospital from January 2021 to December 2021 were divided into control group ( n=251) and hypercalcemia group ( n=36) according to the corrected blood calcium level. The clinical data and serum β-CTX level of the two groups were compared. Logistic regression model was used to analyze the risk factors of hypercalcemia in GD patients. Pearson correlation was used to analyze the correlation between serum β-CTX level and other indexes. Results:Of the 287 GD patients, 36 were diagnosed as hypercalcemia, and the incidence of hypercalcemia was 12.54%. The levels of free triiodothyronine (FT3), free thyroxine (FT4), blood phosphorus (P) and β-CTX in hypercalcemia group were higher than those in control group, and the total parathyroid hormone (iPTH) in hypercalcemia group were lower than those in control group (all P<0.05). Multivariate Logistic regression analysis showed that FT3 ( OR=1.283, 95% CI: 1.049-1.570, P<0.05), iPTH ( OR=0.924, 95% CI: 0.863-0.989, P<0.05), β-CTX ( OR=2.488, 95% CI: 1.193-5.189, P<0.05) were the influencing factors for hypercalcemia in GD patients. Pearson correlation analysis showed that β-CTX was positively correlated with FT3, FT4, blood calcium, P, alkaline phosphatase (ALP), total procollagen type I amino end terminal peptide (PINP), N-bone-gamma-carboxyglutamic-acid-containing proteins (N-MID) and 25(OH)D, and negatively correlated with iPTH (all P<0.05). Conclusions:β-CTX is highly expressed in the serum of GD patients with hypercalcemia, which is a risk factor for the occurrence of hypercalcemia in GD patients.

ABSTRACT

Objective:To investigate the expression of miR-92a-3p in senile osteoporosis (OP) and its diagnostic value in hip fragility fractures.Methods:With the help of the National Center for Biotechnology Information (NCBI) website, the data sets related to OP and miRNA were retrieved and analyzed and screened to obtain the target miRNA. Serum samples were collectedfrom 53 OP patients and 24 healthy people, and the OP patients were divided into hip fragility fracture group (OP_F; n=30) and no hip fragility fracture group (OP_WF; n=23). The subjects' bone mineral density and serum bone metabolism markers were measured: calcium, phosphorus, parathyroid hormone, 25-hydroxyvitamin D, bone alkaline phosphatase, serum C-terminal peptide. The expression levels of target miRNAs in serum samples were detected by qRT-PCR. Chi-square test, t test, Spearman rank correlation and receiver operating characteristic curve (ROC curve) were used to analyze the potential relationship between the expression level of miR-92a-3p and the clinical characteristics of patients.Results:According to the analysis of NCBI website, the expression of miR-92a-3p in OP patients was higher ( t=3.41, P=0.007) than that in healthy people, and the expression level of miR-1304-5p was lower ( t=5.13, P<0.001). qRT-PCR experiments confirmed the above results, and further found that the expression of miR-92a-3p in the OP_F group was significantly higher than that in the OP_WF group (t=3.01, P=0.004), but there was no significant difference in miR-1304-5p between the two groups (t=0.71, P=0.480). Compared with the healthy control group, the BMI ( t=2.71, P=0.008), bone mineral density score ( t=29.02, P<0.001), calcium ( t=61.20, P<0.001), phosphorus ( t=2.54, P=0.013), 25-hydroxyvitamin D (t=3.01, P=0.004) ,bone alkaline phosphatase ( t=12.56, P<0.001), and serum C-terminal peptide ( t=7.52, P<0.001) levels were statistically different between the OP patient group and the healthy control group. Compared with the OP_WF group, the bone mineral density score ( t=2.08, P=0.042), calcium ( t=15.75, P<0.001), bone alkaline phosphatase ( t=2.02, P=0.049) and serum C-terminal peptide ( t=3.39, P=0.001) levels were statistically different between the OP_F group and the OP_WF group. Bone mineral density score and bone alkaline phosphatase concentration were related to the expression of miR-92a-3p and were negatively and positively correlated ( r=0.416, P=0.022), respectively ( r=-0.403, P=0.027). The area under the ROC curve was 0.723, and the level of miR-92a-3p had potential significance in the diagnosis of hip fragility fractures ( P=0.006) . Conclusion:miR-92a-3p is highly expressed in OP and has biological significance for the diagnosis of hip fragility fractures.

Objective@#To explore the clinical value of ultrasound in evaluating the progress and curative effect of knee osteoarthritis, and its correlation with the changes of inflammation and bone metabolism.@*Methods@#30 patients with advanced knee osteoarthritis who were admitted to the Department of Ultrasound, Wenzhou Hospital of Traditional Chinese Medicine, Zhejiang University of Traditional Chinese Medicine from October 2017 to March 2019 were enrolled as non-acute knee osteoarthritis.30 cases of early knee osteoarthritis were included in the same period.Both groups received peripheral blood and musculoskeletal ultrasound on the morning and the 7th and 14th day after admission.The two groups were compared before treatment and treatment.The changes in serum (C-reactive protein) Hs-CRP, (interleukin-6) IL-6 levels in musculoskeletal ultrasound and inflammatory disease activity markers after 7 days of treatment, bone metabolism markers osteocalcin (OC), C Terminal peptide (CTX), and the correlation between serum CTX, OC, Hs-CRP, IL-6 levels and musculoskeletal ultrasound scores in patients with acute knee osteoarthritis.@*Results@#The serum CTX, OC, Hs-CRP and IL-6 levels in the advanced osteoarthritis of knee group CTX[(27.11±3.24)ng/L, (15.98±1.96)ng/L, ( 2.56±1.17)ng/L], OC[(15.47±6.33)mg/L, (11.27±4.14)mg/L, (9.43±2.82)mg/L], Hs-CRP[(14.95±1.44)mg/L, (9.89±1.23)mg/L、(6.26±0.55)mg/L], IL-6[(2.73±0.31)ng/L, (1.38±0.13)ng/L, (1.21±0.25)ng/L]were higher than those in the Early osteoarthritis of knee group before treatment and 7 days and 14 days after treatment CTX[(5.29±1.96 )ng/L, (4.38±1.64 )ng/L, (3.91±1.85 )ng/L], OC[(6.24±1.54)mg/L, (5.62±1.06)mg/L, (5.57±1.40)mg/L], Hs-CRP[(5.78±1.17)mg/L, (4.62±1.02)mg/L, (3.73±0.94)mg/L], IL-6[(0.17±0.15)ng/L, (0.15±0.03)ng/L, (0.15±0.14)ng/L](t=26.634, 8.646, 8.365, 22.256, 7.326, 11.065, 26.945, 6.295, 7.18, 41.433, 6.763, 21.773, all P<0.05). The serum CTX, OC, Hs-CRP and IL-6 levels in the acute knee osteoarthritis group were significantly lower than those before treatment (F=8.262, 7.873, 18.353, 21.45, all P<0.05), non-acute knee There was no significant difference in the above indexes between the osteoarthritis group and the serum at each time point(all P>0.05). The ultrasound score of musculoskeletal in the acute knee osteoarthritis group (9.12±4.27) was higher than that in the non-acute knee osteoarthritis group (3.57±1.82) (t=7.333, P<0.05); serum of patients with acute knee osteoarthritis The levels of CTX, OC, Hs-CRP and IL-6 were positively correlated with the musculoskeletal scores (R2 were 0.848, 0.828, 0.821 and 0.831, respectively, P<0.05).@*Conclusion@#The levels of serum CTX, OC, hs CRP and IL-6 increased with the progress of knee osteoarthritis, and decreased with the prolongation of the time after treatment.The ultrasonic score of knee joint was positively correlated with the changes of the above indexes, and decreased significantly after treatment.The ultrasonic score of knee joint has a certain clinical value in evaluating the condition and curative effect of knee osteoarthritis.
